CNA / PCT Tips for Nurses: How to Treat Nursing Assistants

Nursing assistants play a critical role in the healthcare industry. In the nursing profession, we often refer to nursing assistants as our "eyes and ears." When I'm referring to CNAs or nursing assistants in this video, I'm referring to all types of nursing assistants, including nurses aides, orderlies, nursing assistants, certified nursing assistants (CNA), patient care technicians (PCT), and so forth. Nursing assistants may have different certifications, different scopes of practice, and/or be called different names in different states or territories. These individuals have a physically demanding job, and they are often underappreciated by healthcare staff. As a nurse, it's important that you have a great working relationship with your nursing assistants (CNA, PCT, etc.). Here are some tips to help you do that: 1. Be humble. Just because nurses have the authority to delegate tasks to the nursing assistant, it doesn't mean we're personally superior to them. Treat your nursing assistant with respect, just as you would another nurse or doctor. 2 Make them feel like they are a part of the team. Nursing assistants can gather important information as they care for patients. By having open communication with your CNA or PCT, you can gain critical insights that can help you provide better patient care. 3. Be a servant leader. If you see a nursing assistant struggling with patients, step up and give them a hand (if you can). It's easy for them to get overworked and overwhelmed. By helping them out, they'll be much more likely to return the favor, and the patient will receive great care. 4. Thank them. The nursing assistants sometimes get overlooked by patients and other members of the healthcare team. If you see a CNA or PCT that is working hard and going above and beyond, take time to recognize that by telling the nursing manager, giving them a recognition card, or talking to other nurses about buying a gift card (or lunch). Nursing assistants provide a critical service to patients, and as a nurse, I've always been thankful to have nursing assistants by my side. By taking the time to treat them with respect, communicating with them, leading them, and thanking them, we can foster a great working relationship that will benefit everyone. New nurse graduates will experience "new nurse anxiety" during some point while on the job. It is most common to experience anxiety as a new nurse during the first 6 months to year of being a nurse. The anxiety seems to start at the beginning of orientation (because you are new) then decrease during orientation and peaks when orientation is over (when you're on your own as a nurse). In this video, I'm going to talk about what causes new nurse anxiety (feeling overwhelmed with the new environment/people, feeling not confident in performing skills, harming a patient unintentionally, dealing with chaos like balancing patient loads, completing paperwork, communicating with members of the healthcare team etc.) and how to deal with anxiety so you don't become burnt out and possibly leave the nursing profession. According to the ANA these are 3 major reasons why this is a bad piece of legislation. And we talk thoroughly about each of these reasons and so much more. 1. They are not based on any valid science. There are no studies that provide evidence that staffing ratios alone will improve quality of care. In fact, many of the studies that the union cites the most say the exact opposite. 2. The ratios are the same for each unit in every hospital on both night and day shifts — from the largest teaching hospitals in Boston to smaller, rural hospitals in Western Massachusetts. But each hospital is unique, each unit is different, each nurse is different and each patient population is different. 3. The mandate does not account for the acuity of the patient or the experience of nurses working on the unit at that time. Do nurses really have to clean poop? This is a common question that some pre-nursing students (or those interested in becoming a nurse) ask me. The answer is…YES! Cleaning poop (stool) is definitely a part of a nurse’s job. It’s not the most glamorous part of the job, but it is a very important part of providing patient care. As a nurse, our job is to take care of our patients, and bodily fluids definitely come with the territory. How Often Do Nurses Clean Stool (Poop)? So, how often does a nurse have to clean stool on his or her shift? That really depends on a lot of factors. First, it can depend on the nurse’s specialty. There are some specialties (or areas of nursing) where stool encounters will be less common. A person who works as a nurse manager, for example, really deals with the business side of nursing. Their encounters with stool will be relatively infrequent (or almost never), depending on where they work. In contrast, a nurse who is working on the floor of a hospital, who has a load of patients, may have to deal with stool during nearly every shift (or even multiple times per shift). How often you clean stool can also depend on the health status of your patients during each shift. For example, if you work on a hospital floor and have a patient with an ileostomy, which puts out stool frequently, you’ll have to assist the patient with ostomy bag changes and cleaning the stoma. Or, if you’re prepping a patient for an in-patient colonoscopy, your patient will be given a solution to help cleanse the colon, so you’ll be dealing with stool in that situation. In addition, if you’re caring for a patient with a GI infection such as C. diff, the patient will likely have issues with incontinence and diarrhea. If you work as a labor and delivery nurse, you may encounter stool if a woman has a bowel movement during labor, or while you are assisting with changing the newborn’s diaper. So, the amount of stool you’ll encounter as a nurse depends a lot on your specialty and the health status of your patients. When you start your clinical rotations in nursing school, you’ll get a good idea of how often you’ll have to encounter stool in the various areas of nursing. Cleaning Stool is a Team Effort in Nursing An important thing to remember is that cleaning stool is really a team effort. A lot of people have the idea that cleaning stool is the nursing assistant’s job (or CNA), but it’s not. Many times, you’ll have a patient who is immobile or incontinent, and you’re going to need help turning the patient to clean them. You may also need help with giving a bed bath or changing linens. It’s definitely a team effort. In my career spanning half a decade, I have both been the interviewee and interviewer! And I can certainly give you a few nurse interview tips :) First of all, being prepared for the nursing interview is key... but how do you prepare?? The first thing that you need to do is get comfortable talking about yourself. Which is harder than it sounds, especially with a strangers face staring back at you. However, if you know this ahead of time, you can prepare a monologe that will show that you are confident and will be a competent nurse! the nurse interview questions are usually really really hard, with tons of open end questions and also questions that make you talk about your weaknesses. You need to be honest here, talking about your failures sounds like you are saying that you are weak... but you have to focus on the valuable lesion learned and how you will improve yourself in the future!! Nursing interview, I believe you will literally bomb if you do not prepare in at least one way or another... Donning and removing (doffing) PPE (personal protective equipment) such as gloves, gown, mask/respirator (N95 mask), and goggles/face shield. In this video, I will demonstrate how to don (putting on) and doff PPE in preparation for the NCLEX exam. You will learn the sequence of donning (applying) PPE (Hand Hygiene, Gown, Mask/Respirator, Goggles/Face Shield, Gloves), and the sequence of taking off or doffing personal protective equipment (Gloves, Goggles/Face Shield, Gown, Mask/Respirator, Hand Hygiene). When applying and removing PPE it is important to remember the following highlights: don the PPE outside the patient's room, when in the patient's room keep hands away from face and don't touch your PPE, try to minimize touching unnecessary areas in the patient's room to avoid contamination, remove PPE at the patient's doorway or outside the room and perform hand hygiene immediately, always remove respirator (if worn) outside the patient's room after closing the patient's door, and if at any time your hands become contaminated while removing articles of PPE perform hand hygiene before proceeding with removing other articles of PPE. Why do I suddenly have Adele's "Hello" stuck in my head? Oh, because "I must have called (my coworkers) a thousand times." Pretty sure they've started to pretend they don't hear me! During my last shift I must have gowned-up over 100 times (or, at least it felt that way!) Seriously, most of my shifts are spent in isolation rooms - a lot of patients are on contact precautions nowadays. Let's be real, I dread having to gown-up then undress and gown-up again just to grab a darn alcohol swap. Why do I ALWAYS forget those? Ugh. In 2018 I have set the resolution to be better prepared for the room of no return. For a western nurse, one of the challenges of hospital practice is to use equipment and techniques that are different. In Nepal, the hospitals minimize the amount of disposable supplies. The principles of sterile technique are still applied, but the tools are not the same. It's been described as entering an alternate universe of hospitals, inasmuch as the sorta-similar-but-not-quite aspect is repeatd for many of the small clinical systems that become second nature after awhile of practice in a western hospital. Failing to Think Critically: When you're a new nurse, it's easy to focus solely on tasks that need to be completed. Instead, you have to force yourself to think critically, as I describe in this video. 2. Social Media Problems: A common mistake that new nurses make is posting sensitive patient information on social media. Remember, we have HIPAA and other privacy laws that we must obey. In addition, many nurses post too often on social media during a shift, which can affect patient care. 3. Time Management Problems: It can be a real challenge to complete all of your tasks as a new nurse, and if you don't take time to learn how to perform your tasks more efficiently, things will only get worse. According to the ANA these are 3 major reasons why this is a bad piece of legislation. And we talk thoroughly about each of these reasons and so much more. 1. They are not based on any valid science. There are no studies that provide evidence that staffing ratios alone will improve quality of care. In fact, many of the studies that the union cites the most say the exact opposite. 2. The ratios are the same for each unit in every hospital on both night and day shifts — from the largest teaching hospitals in Boston to smaller, rural hospitals in Western Massachusetts. But each hospital is unique, each unit is different, each nurse is different and each patient population is different. 3. The mandate does not account for the acuity of the patient or the experience of nurses working on the unit at that time.
